Inclusion criteria

Inclusion criteria: 1.sleep disorders, hypertension, hyperlipidemia, diabetes, coronary heart disease, neurodegenerative diseases, and other chronic diseases =2 types; 2.the age must be 60 years or older; 3.Taking 5 or more types of medication; 4.regardless of gender;

Exclusion criteria

Exclusion criteria: 1.Those who cannot obtain complete medication information; 2.Those with a life expectancy of less than one year; 3.There are not any other researchers who believe there might be factors that could interfere with participating in the trial study or evaluation;
