Inclusion criteria
Inclusion criteria: Patient inclusion criteria: (1) diagnosed with heart failure according to the Chinese Guidelines for the Diagnosis and Treatment of Heart Failure 2018; (2) age = 18 years; (3) at least one caregiver with primary caregiving responsibility; (4) able to complete the questionnaire in writing or verbally; (5) informed consent and voluntary participation in this study. Inclusion criteria for caregivers: (1) patient's designated primary caregiver; (2) age =18 years; (3) caregiver with the longest caregiving time and participation in the patient's medical decision-making; (4) informed consent and voluntary participation in this study. Moreover, at least one of the patients with heart failure and their caregivers used a smartphone and was proficient in operating WeChat, and neither of them had recently participated in other intervention studies.
Exclusion criteria
Exclusion criteria: Patient exclusion criteria: (1) acute myocardial infarction within the past month; (2) other systematic serious comorbidities or complications, such as malignant tumors, liver, kidney, lung and other organs failure, mental disorders and so on. Exclusion criteria for caregivers: (1) receiving care remuneration; (2) having serious physical diseases (such as malignant tumors, vital organ failure, etc.), cognitive disorders, or mental illnesses.
