Inclusion criteria
Inclusion criteria: 1. Patients diagnosed with type 2 diabetes according to the WHO diagnostic criteria in 1999, must have at least one or more chronic diseases such as hypertension, hyperlipidemia, coronary heart disease, gout, chronic osteoarthritis, etc. 2. HbA1c level is equal to or greater than 7.0%. 3. Willing to sign a contract with a pharmacist for home pharmaceutical services. 4. The patient or family members are conscious, have basic language communication skills, and can use a smartphone. 5. Age between 18 and 80 years old, with an expected survival period of more than six months.
Exclusion criteria
Exclusion criteria: 1. History of mental illness, cognitive dysfunction, or communication barriers. 2. Severe complications leading to prolonged hospitalization or death. 3. Long-term bedridden, lack of ability to take care of oneself.
