Inclusion criteria
Inclusion criteria: 1.18 years old or above; 2. diagnosed with type 2 diabetes; 3. Hemoglobin A1C >=7.5%; 4. able to use smartphone; 5. able to walk; 6. have basic Chinese reading and writing skills; 7. do not participate in other type 2 diabetes management program at the same time
Exclusion criteria
Exclusion criteria: 1. severe sensory or motor disorders, or mental disorders; 2. physical conditions are not suitable for basic exercises such as walking; 3. is participating in other diabetes self-management program
