group1:Intervention time: The first to 8th month Intervention: The "Double-S" consists of five major components: electronic health education manual, MG animation library, SMS library, wechat Q&A group
MG animation Library: Researchers send an MG animation to wechat group every Tuesday and Friday
SMS library: Researchers send SMS messages to wechat group every Monday, Wednesday, Saturday and Sunday (1-2 messages/day)
Wechat Q&A Group: Every Friday from 14:00-15:00, researchers answer questions for patients
Material incentive package: sent by the investigator to the patient during implementation

60 Years to 75 Years

Inclusion criteria

Inclusion criteria: 1. 60 years old <= age < 75 years old; 2. The patient was diagnosed with T2DM by hospital; 3. Permanent residents of the community (living in the area for more than half a year); 4. Can use mobile phone wechat; 5. Volunteer to participate in this study

Exclusion criteria

Exclusion criteria: 1. Communication barriers, unable to complete the investigator; 2. Mental disorders; 3. physical disability resulting in inability to stand; 4. Complicated with other serious diseases (such as severe heart, liver, renal insufficiency, respiratory failure and malignant tumors)
