Inclusion criteria

Inclusion criteria: ?Age =18 years;?the patient was diagnosed by physician,meeting the latest guidline of ischemic stroke comfired diagnosis,without TIA;?NIHSS=15,having the upper limb functional impairment;? Glasgow Coma Scale=14,MoCA-5 min=12;have ability to communicating fluently in the research;?having smart phone and covered with wifi at home ?accomplaining with a care-giver?Informed consent obtained.

Exclusion criteria

Exclusion criteria: ?with unstable vital signs or progressive stroke ?having other kinds of diseases:severe cardiovascular disease,liver disease;kindney disease;the recent history of craniocerebral trauma, bone and jiont injury, major surgery;or cancer ?having severe congnition impairment,mental illnesses or psychosis disease; ?having severe speech communication impairment;visual impairment or hearing impairment ; ?those whose home care-givers are reluctant to participante in the patient's rehabilitation progress.
