Inclusion criteria
Inclusion criteria: (1) aged 45-80 years; (2) suffering from two or more non-communicable diseases, with a hypertension diagnosis that meets the criteria for essential hypertension as set out in the Chinese Guidelines for the Prevention and Control of Hypertension (2024 Revision) / diabetes mellitus diagnosis that meets the criteria for type 2 diabetes mellitus as set out in the 2024 ADA Diabetes Medical Diagnostic and Treatment Criteria; (3) use of more than five medications; (4) giving informed consent to participate in the present survey, the mentally alert and able to communicate effectively.
Exclusion criteria
Exclusion criteria: (1) had psychological and psychiatric disorders or other mental health conditions requiring medication; (2) were diagnosed with New York Heart Association class III or IV heart failure; (3) had critical illnesses such as severe hepatic or renal insufficiency or malignant tumors, and were unable to tolerate the assessment process and its effects; (4) could not be contacted at the time of the initial telephone follow-up and had no other contact information. Patients who died or had no complete clinical information were also excluded.
