Interventions

Experimental group:AI Health Education Precision Linkage System
Control group:Control group participants will receive usual medical care by a multidisciplinary treatment team. Services that each participant receives involve: 1) periodic physician consultations for
2) follow-up visits at predetermined intervals to review disease status
3) routine diagnostic assessments to check health markers
4) systematic health education sessions on managing their condition, treatment compliance, and changes in lifestyle
and 5) specialist consultations from related medical providers if there are indications.

Inclusion criteria

Inclusion criteria: 1.Age >= 18 years. 2.Patients diagnosed with COPD according to GOLD (Global Initiative for Chronic Obstructive Lung Disease) criteria. 3.No acute exacerbation or other major health problems in the past month to ensure the stability of the disease state during the intervention. 4.Be able to operate a smartphone proficiently and have the habit of using WeChat so as to be able to accept and participate in the WeChat-based intervention program smoothly. 5.Willing to participate in the study and sign the informed consent form before the start of the study.

Exclusion criteria

Exclusion criteria: 1.Those with severe mental illness and impaired cognitive function who are unable to cooperate with the study. 2.Those who do not know how to use a smartphone. 3.Those who have participated in related research before.
