Inclusion criteria
Inclusion criteria: Patients aged 40 years and above; Newly diagnosed stable COPD patients in grassroots units; Simultaneously coexisting with hypertension, hyperlipidemia, or hyperglycemia, one, two, or three; Patients who can cooperate with grassroots units in chronic disease management; Patients who are willing and able to sign written informed consent.
Exclusion criteria
Exclusion criteria: Patients currently diagnosed with asthma; Patients with chronic obstructive pulmonary disease (COPD) and other major diseases (such as cancer) related to the three highs; Patients who are also participating in other clinical studies;
