Intervention group:Empowerment pulmonary rehabilitation self-management intervention once every 2 weeks for 12 weeks after discharge
Control group:Three times, once a month after discharge, lung rehabilitation education and follow-up will be led by nurses

Inclusion criteria
Inclusion criteria: 1. Diagnosed as COPD grade II-IV patients according to the COPD Global Initiative guidelines; 2. Preliminary screening using Fried frailty phenotype shows at least one abnormality; 3. Patients and their families have smartphones and can use WeChat; 4. Clear consciousness, normal communication skills, and willingness to cooperate.
Exclusion criteria
Exclusion criteria: 1. Patients with severe cardiovascular, cerebrovascular, liver and kidney diseases; 2. Patients with combined active pulmonary tuberculosis, localized pulmonary bulla, malignant tumors, hyperthyroidism, severe hearing and vision impairment, and limb movement disorders; 3. Patients with systolic blood pressure = 180mmHg and/or diastolic blood pressure = 110mmHg.
