Inclusion criteria

Inclusion criteria: 1.Patients with a clinical diagnosis of ischemic heart disease, stroke, arrhythmia, heart failure, or hypertension. 2.Age = 65 years, no gender restrictions. 3.Both the participant and their family are informed about the study and voluntarily agree to participate. 4.The elderly participant or their legally authorized representative signs the informed consent form.

Exclusion criteria

Exclusion criteria: 1.ADL (Activities of Daily Living) score = 60. 2.Presence of severe cognitive impairment, a history of severe mental illness, severe visual, hearing, or communication impairments, severe multi-organ failure, or terminal illness with a life expectancy of = 1 year. 3.Inability to cooperate with digital services. 4.Poor compliance. 5.Other conditions that the researchers deem unsuitable for participation in the study.
