Inclusion criteria: 1.Age =60 years old; 2.Clear awareness, barrier-free communication; 3.Diabetic patients in the community;

Exclusion criteria

Exclusion criteria: 1.Have left the registered home address; 2.Life expectancy, as judged by community doctors, is < 3 months;
