Postoperative delirium

Interventions

Experimental Group 1:Intranasal insulin administration at a dose of 40 IU per session (0.5 ml), twice daily at 9:00 AM and 7:00 PM.
Experimental Group 2:Intranasal insulin administration at a dose of 20 IU per session (0.5 ml), twice daily at 9:00 AM and 7:00 PM.
Control group:Intranasal administration of 0.9% saline solution (0.5ml) twice a day at 9:00 AM and 7:00 PM.

60 Years to 100 Years

Inclusion criteria

Inclusion criteria: 1. Age = 60 years 2. American Society of Anesthesiologists (ASA) classification of physical status I-III 3. Diagnosed with traumatic hip fracture upon admission 4. Capable of accurately understanding the Mini-Mental State Examination (MMSE), with an admission MMSE score greater than 23.

Exclusion criteria

Exclusion criteria: 1. Contraindications to intranasal administration, such as nasal defects or lesions 2. History of insulin allergy 3. History of alcohol abuse 4. Inability to communicate prior to surgery, due to conditions such as coma, severe dementia, language disorders, or significant visual or auditory impairments 5. Long-term use of antidepressants or sedatives 6. Presence of systemic diseases that affect cerebrospinal fluid biomarker levels 7. Contraindications to spinal anesthesia puncture 8. History of neurological or psychiatric disorders 9. Presence of delirium or subsyndromal delirium upon admission.
