Inclusion criteria

Inclusion criteria: (1) The primary diagnosis is a cardiovascular disease (such as coronary heart disease, heart failure, hypertension, arrhythmia, valvular heart disease, cardiomyopathy, pericardial disease, pulmonary heart disease, congenital heart disease that still requires treatment or surgery in adulthood, etc.); (2) Age is 60 years or older; (3) Currently hospitalized for at least 3 days and has a certain understanding and adaptation to the hospital environment; (4) Informed consent, voluntarily participating in the survey; (5) Conscious and able to understand and communicate in Chinese; (6) Own and know how to use a smartphone; (7) Good vision and hearing, able to complete the questionnaire independently or with the assistance of the surveyor.

Exclusion criteria

Exclusion criteria: (1) Accompanying severe mental illness (such as schizophrenia, major depression, etc.) or cognitive dysfunction (such as Alzheimer's disease); (2) Current vital signs are unstable, making it unsuitable to participate in the survey; (3) Presence of severe non-cardiovascular diseases (such as advanced cancer, acute renal failure, severe liver dysfunction, etc.); (4) Life expectancy is less than 3 months; (5) Currently participating in other research studies.
