Inclusion criteria
Inclusion criteria: Patients who met the following conditions at the same time : 1 age greater than or equal to 60 years old, less than 80 years old ; 2 course of disease = 10 years ; 3 Patients with a clear diagnosis of type 2 diabetes can be combined with hypertension, coronary heart disease, hyperlipidemia and other chronic diseases ; 4 with self-care ability, and can clearly communicate with the team ; 5 signed informed consent, voluntary participation.
Exclusion criteria
Exclusion criteria: Any of the following points can be excluded : 1 patients with type 1 diabetes ; 2 patients with impaired glucose tolerance, patients with impaired fasting glucose ; 3 patients with severe diabetic complications, mental illness, malignant tumor ; 4 patients with cognitive dysfunction, mental and language disorders ; 5. researchers who are participating in or have participated in other projects in the past 30 days.
