Inclusion criteria
Inclusion criteria: Patient Section: i) Meeting the international clinical criteria for knee osteoarthritis (OA): Age 45 or older; Knee pain associated with activity; Morning knee stiffness lasting no longer than 30 minutes; ii) A history of knee pain for at least 3 months; iii) Report of knee pain on most days in the past month; iv) An average pain score of at least 4 out of 11 on a numerical rating scale during walking in the past week; v) Reporting difficulty with walking and climbing stairs; vi) Ability to use an internet-connected device; vii) Willingness to participate in non-surgical, non-pharmacological services that combine remote diagnosis and community-based rehabilitation; viii) Passing a pre-exercise screening questionnaire (consisting of 7 questions with "Yes" or "No" answers, designed to identify individuals with potential disease symptoms or signs, or those at higher risk of adverse events during exercise). Healthcare Service Provider Section: General requirements: Registered healthcare practitioners engaged in services related to osteoarthritis, able to use an internet-connected computer, willing to complete training if randomly assigned to the intervention group, willing to report all professional development activities participated in during the intervention period, and able to provide informed consent and fully commit to the study process. Specifically: a) Community Health Center Staff: At least one senior medical staff member from the department agrees to participate; The center has internet platform tools for joint decision-making with higher-level hospitals; Possesses current health insurance and meets hospital referral conditions; The center agrees to randomization and subsequent follow-up surveys. b) Hospital Clinicians: No conflicts of interest or personal benefit from the medical service practice; Holds a valid clinical practice license; Willing to conduct clinical practice research according to the study protocol.
Exclusion criteria
Exclusion criteria: Exclusion criteria for patients: Waiting for/planning joint surgery within the next 12 months; Previous joint replacement surgery on the affected knee; Recent knee surgery (within the past 6 months); Alternative diagnoses for knee osteoarthritis, such as referred pain from the spine or hip; Secondary arthritis, such as inflammatory or post-traumatic arthritis; Unable to comply with the study protocol due to conditions such as cognitive impairment, inability to read or speak; Patients who use a wheelchair; Self-reported inflammatory arthritis (e.g., rheumatoid arthritis); Any neurological condition affecting the lower limbs; Any unstable or uncontrolled cardiovascular disease. Anyone meeting the following conditions must obtain permission from a general practitioner to participate: i) Reported falls (in the past 12 months) or housebound due to mobility issues; ii) Failed the initial stage of pre-exercise screening questions. After meeting the inclusion criteria, there are no special exclusion criteria for doctors.
