Muscle atrophy in critically ill patients on mechanical ventilation

Interventions

SC:According to the current nutritional guidelines, 25kcal/kg/d is recommended as the calorie target, while the protein target is 1.2g/kg/d.
IC:The calorie target is based on the actual resting energy expenditure detected by an indirect calorimetry analyzer, and the protein target is 1.2g/kg/d

Inclusion criteria

Inclusion criteria: 1) Age = 18 years old; 2) Respiratory failure caused by various reasons requires tracheal intubation and mechanical ventilation for at least 7 days, with FiO2 = 60%; 3) NRS 2002 = 3 points upon admission to the ICU, requiring nutritional support and treatment; 4) The condition is reversible.

Exclusion criteria

Exclusion criteria: 1) Patients with neuromuscular diseases, hyperthyroidism, and tumors; 2) Existence of cervical spinal cord injury; 3) Severe chest deformity combined without surgical repair, resulting in respiratory function limitation; 4) The respiratory state is unstable and requires frequent adjustment of ventilator parameters; 5) Combined moderate to severe liver dysfunction; 6) Use of glucocorticoids or continuous use of neuromuscular blockers during mechanical ventilation maintenance; 7) Pregnancy; 8) The clinical doctor in charge of treatment believes that this intervention is not in the best interest of the patient.
