Inclusion criteria

Inclusion criteria: ?age =60 years; ? screened as older people with reduced intrinsic capacity in the community or with reduced intrinsic capacity in the community to the extent of needing care and support from others according to the ICOPE screening tool; ? gave informed consent to voluntarily participate in the study; ? able to communicate normally, with the ability to read and comprehend text; and ? life expectancy of not less than 24 months.

Exclusion criteria

Exclusion criteria: ?community residence for less than 6 months; ? critical condition, or serious mental illness.
