Intervention group:When a doctor serve a patient, he or she need to apply the patient triage tool in rehabilitation.
Control group:Carry out the routine diagnostic and treatment activities without the use of the patient triage tool

Inclusion criteria

Inclusion criteria: (1) Patients: ?had rehabilitation need; ?can complete the evaluation; ?agree to participate the study. (2) Doctors: ?had qualified certification; ?had clinic experiences; ?agree to partipate the study.

Exclusion criteria

Exclusion criteria: (1) Patients: ?only help family member to take medcine; ?can not complete the evaluation during the study. (2) Doctors: ?off the work; ?retired doctor.
