Control Group:Routine care
Intervention Group:On the basis of the control group, the intervention program for the frailty of elderly stroke patients based on HAPA theory and HPM was implemented in the intervention group.

Inclusion criteria

Inclusion criteria: (1) Age =60 years; (2) Compliance with the diagnostic criteria established in the Chinese Guidelines for the Diagnosis and Treatment of Cerebral Haemorrhage (2019) and the Chinese Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke (2018), with clinical cranial MRI or CT confirming the diagnosis; (3) Patients with mild-to-moderate stroke scored <15 points according to the National Institutes of Health Stroke Scale (NIHSS); (4) Tilburg Index of Frailty Scale =5 points; (5) Signed informed consent to participate in this study.

Exclusion criteria

Exclusion criteria: (1) Patients who are unable to complete an exercise programme due to severe movement disorders; (2) Presence of significant communication disorders and speech disorders; (3) History of psychiatric or psychological disorders; (4) Patients with co-morbid serious illnesses, such as severe anaemia and malignant tumours.
