Inclusion criteria

Inclusion criteria: (1) Age of 60 years or older; (2) Diabetic patients who meet the diagnostic criteria outlined in the "China Guidelines for the Prevention and Treatment of Type 2 Diabetes Mellitus (2020 Edition)" and have been diagnosed with type 2 diabetes mellitus by a specialist physician; (3) A confirmed diagnosis of diabetes for 3 months or longer; (4) Long-term residents of the community (holding a local residency permit or having resided in the city for 6 months or more continuously); (5) Possessing clear consciousness, normal communication skills, auditory and visual abilities, and the ability to comprehend and cooperate with the investigation; (6) Individuals who provide informed consent and voluntarily agree to participate in the survey.

Exclusion criteria

Exclusion criteria: (1) Patients with acute diabetic complications such as Diabetic Ketoacidosis (DKA), Hyperosmolar Hyperglycemic Syndrome (HHS), lactic acidosis, etc.; (2) Patients with severe chronic diabetic complications or comorbidities, including acute myocardial infarction, uncontrolled stage 3 hypertension (systolic =180mmHg and/or diastolic =110mmHg), renal failure with an estimated glomerular filtration rate (eGFR) < 60ml/min/1.73m2, etc.; (3) Patients with moderate to severe dementia (e.g., Mini-Mental State Examination (MMSE) score = 20) and/or those with severe mental or psychological illnesses.
