Inclusion criteria

Inclusion criteria: 1. In the fourth national cerebrovascular disease conference by diagnostic criteria, diagnosed by MRI or CT examination of ischemic cerebral apoplexy; 2. First onset, age =60 years old, and the course of disease was 2 weeks to 6 months; 3. Patients who can walk alone or with assistive devices; 4. Low physical activity level assessed by IPAQ; 5. The home rehabilitation of patients after discharge; 6. Home Internet access and smartphone use by the patient or caregiver; 7. Patients with voluntary participation and sign the informed consent

Exclusion criteria

Exclusion criteria: 1. Simple Mental State Examination (Mini - getting the State Examination, MMSE) 27 points or less; 2. With severe aphasia, communication disorders of patients; 3. with severe heart, lung, liver, kidney disease and malignant tumor patients; 4. At the same time to participate in other studies; 5.
