Inclusion criteria
Inclusion criteria: 1. Meeting the diagnostic criteria for heart failure patients in the 2018 Chinese Guidelines for the Diagnosis and Treatment of Heart Failure upon admission; 2. Cardiac failure as the primary admission diagnosis; 3. Age = 60 years old; 4. Ability to read and express oneself in language, with no communication barriers.
Exclusion criteria
Exclusion criteria: 1. Having experienced myocardial infarction, stroke, chronic obstructive pulmonary disease, renal failure, cancer, etc. in the past 3 months (combined with the above diseases, the symptoms of heart failure patients have a compound effect); 2. Intending to enter a care facility after discharge; 3. having participated in intervention trials other than routine care in the past three months.
