Perioperative lung injury

Interventions

Traditional lung protection group:Tidal volume set at 8 mL/kg PBW, PEEP at 8 cmH2O
Driving-pressure guided group:Adjust tidal volume between 4-8 mL/kg PBW to maintain driving pressure between 9±1 cmH2O, with PEEP set at 8 cmH2O.

Inclusion criteria

Inclusion criteria: 1. Patients undergoing elective lung wedge resection, lobectomy, or segmentectomy (with single-lung ventilation time > 1 hour). 2. Age = 18 years. 3. Signed informed consent form.

Exclusion criteria

Exclusion criteria: 1. ASA classification IV or higher. 2. NYHA functional class III or higher. 3. Severe respiratory diseases such as bullous emphysema, pneumothorax, COPD requiring oxygen therapy, and prior thoracic chemotherapy. 4. Pregnancy or lactation. 5. Enrollment in other studies. 6. Patient refusal to participate in this study. 7. BMI > 35 kg/m². 8. FEV1 less than 50% of predicted. 9. Coagulation disorders. 10. Use of immunosuppressive drugs within the three months preceding surgery.
