60 Years to 90 Years
Inclusion criteria
Inclusion criteria: ? Age = 60 years old; ? At least one chronic disease; ? Duration of illness not less than one year; ? Informed consent and voluntary participation in this study; ? Conscious enough to complete the survey independently or with the assistance of the researcher; ? Permanent resident of the community (domicile in the city, or long-term residence for 6 months or more).
Exclusion criteria
Exclusion criteria: (i) in critical or terminal condition; (ii) unable to take care of themselves; (iii) participating in other studies.
