Inclusion criteria

Inclusion criteria: 1) Patients aged = 55 with Type 2 diabetes, cognitive impairment, muscular dystrophy, frailty, motor cognitive risk syndrome, malnutrition, or any other geriatric syndrome. 2) Own a smartphone and have the ability to view and send messages through "WeChat" or similar mobile applications. 3) Able to participate in a 6-month health management and follow-up program. 4) No severe cognitive impairment. 5) No visual impairment that hinders the use of a smartphone. 6) Currently not enrolled in any other health management programs.

Exclusion criteria

Exclusion criteria: 1) Patients with severe illnesses (in ICU, in advanced stages of disease, severe dementia, severely dependent) or critical condition patients. 2) Elderly individuals who are unable to communicate effectively through verbal language. 3) Patients with mental disorders or other conditions that prevent them from signing an informed consent form. 4) Individuals who refuse to participate in the survey. 5) Patients who are unable to use AI devices or are unable to perform activities of daily living.
