Inclusion criteria

Inclusion criteria: 1.Community-dwelling adult (aged >=18 years); 2.Diagnosis of stroke; 3.Diagnosis of hypertension; 4.Able to communicate in Mandarin or Cantonese; 5.Has a smartphone that can download instant messaging applications, such as WeChat; 6.Has an adult family member involved, willing to take care of the patient's diet, able to communicate in Mandarin or Cantonese, and has a smartphone that can download instant messaging applications; 7.Able to participate in this study for 4 months follow up; 8.Willing to sign the informed consent form.

Exclusion criteria

Exclusion criteria: 1.Currently receiving stroke active care in acute phase in inpatient settings; 2.Currently participating in any types of dietary interventions or other HTN-related lifestyle modification interventions; 3.Inability to consume food orally (e.g., using esophageal feeding tubes or other types of enteral nutrition); 4.Has mental disorders.
