Experimental group:When the intervention group patients are about to be discharged, the attending physician informs the patients that they need to continue monitoring their blood pressure for 90 days.

Inclusion criteria
Inclusion criteria: 1.Patients with first-ever ischemic stroke confirmed by head CT or MRI examination, in accordance with the revised diagnostic criteria for ischemic stroke from the 4th National Cerebrovascular Disease Conference. 2.Patients meeting the diagnostic criteria for hypertension in the "Chinese Guidelines for the Prevention and Treatment of Hypertension (2018 Revised Edition)." 3.Adult patients aged 18 years or older. 4.Patients capable of independently or with assistance measuring and recording blood pressure. 5.Patients who have signed informed consent.
Exclusion criteria
Exclusion criteria: 1.Patients unable to wear blood pressure monitoring devices on the upper arm. 2.Patients residing outside of the main urban areas of Beijing or with unstable residency. 3.Patients with subclavian steal syndrome or vertebral artery stenosis. 4.Patients with major diseases such as heart, liver, kidney, malignant tumors, or severe psychological disorders such as anxiety or depression. 5.Patients unable to commit to completing the study as required and attending three follow-up visits/reviews within 90 days or returning the study equipment after the study ends.
