18 Years to 80 Years

Inclusion criteria

Inclusion criteria: a. Meets the diagnostic criteria for chronic heart failure, and the New York Heart Association (NYHA) cardiac function grading is II - III; b. Age range from 18 to 80 years old; c. Living in a research community; d. Can use a phone or mobile phone.

Exclusion criteria

Exclusion criteria: a. Cardiac function level IV; b. The current very weak health condition; c. Severe mental illness or cognitive impairment; d. Late stage cancer; e. Illiteracy; f. Hearing and speaking disorders; g. Abuse of drugs or alcohol; h. Has undergone heart surgery in the past 6 weeks; i. Currently involved or have participated in other heart failure health management projects or similar projects in the past.
