Inclusion criteria

Inclusion criteria: (1) For patients: ? diagnosis of coronary atherosclerotic heart disease; ? about to be discharged from the hospital into the home environment; ? use of smart mobile communication devices such as smart phones; and ? informed consent to voluntarily enroll in this study. (2) For carers: ? living with the patient; ? playing a primary caregiving role; ? using smart mobile communication devices such as smartphones; and ? giving informed consent and voluntarily joining the study.

Exclusion criteria

Exclusion criteria: (1) For patients: ? In an acute stage of illness or in a serious condition that makes participation unsuitable; ? with mental illness; ? with impaired consciousness; ? speech and hearing impairment. (2) For carers: ? speech and hearing impairment; ? employment relationship with the patient.
