Inclusion criteria

Inclusion criteria: (1) Patients with the diagnosis of essential diabetes contracted by a community family doctor in Chengdu who are taking 3 or more drugs at the same time, or combining drugs with other diseases; (2) Age 35-75 years, disease duration > 1 year, gender is not limited; (3) Patients who sign the informed consent form and require the patient not to change the drug or stop the drug without authorization during the implementation of drug treatment management, and accept regular follow-up. (4) Patients who are proficient in operating smartphones.

Exclusion criteria

Exclusion criteria: (1) Patients who do not meet the inclusion criteria of the study subjects, such as patients with secondary hypertension and diabetes; Patients with acute myocardial infarction and acute phase of stroke (< 3 months); 2) Patients who are not easy to cooperate (obvious impairments of intellectual, hearing, and limb movements); 3) Patients with severe illness and life expectancy of less than 1 year; (4) Patients who are unable to participate in this project due to mental disorders, severe activity disorders and other diseases; (5) Patients who are participating in similar research projects in this project; (6) Patients who do not comply with the interventions applied in this project.
