Inclusion criteria
Inclusion criteria: 1. Community: permanent population > 10,000, and there is an independent community health service center; 2. = 60 years old 3. who can move autonomously with or without the help of a walker 4. Illiterate and visually impaired need to have caregivers, and the caregivers are willing to cooperate with the research 5. The long-term caregiver has certain comprehension ability, literacy and reading ability 6. The person or long-term caregiver has a smartphone and has the ability to use the APP 7. Be conscious,and sign the informed consent form.
Exclusion criteria
Exclusion criteria: 1.People with severe heart, liver, and kidney dysfunction ,malignant tumor disease, cognitive impairment, or long-term bed rest, long-term dependence on wheelchair activities. 2. Participate in other forms of platform management or clinical trials for fall management within one month.
