Inclusion criteria: (1) Age between 40-75 years; (2) Informed consent to participate in this survey

Exclusion criteria

Exclusion criteria: Subjects suffering from serious mental illness, speech impediment or mental deficiency, or considered by the investigator to be unsuitable to conduct the survey.
