Inclusion criteria

Inclusion criteria: ? Patients who meet the diagnostic criteria of type 2 diabetes; ? Adult patients aged = 18 years old; ? Patients who have primary school or above education level; ? Patients or family members have smartphones and can use WeChat; ? Patients who registered and managed in the hospital, with no plans to go out in the past six months; ? Patients who have clear expression ability; ? Patients who have complete cognitive and behavioral abilities; ? Patients who sign the informed consent form and voluntarily participate in the study;

Exclusion criteria

Exclusion criteria: ? Type 1 diabetes patients or pregnancy diabetes; ? Patients who are non-cooperative, unconscious, inarticulate in language and difficult to communicate; ? Patients with a history of mental illness or mental illness; ? Patients with severe and unstable heart, lung, liver and other diseases that affect their ability to take care of themselves; ? Pregnant or lactating women; ? Patients whose family members are unwilling to support; ? Patients refuse to communicate through WeChat; ? Patients who are participating in similar studies;
