Group T:Anesthesia induction: Esketamine (0.25mg/kg) Anesthesia maintenance: Esketamine 0.1mg/kg/h continuous infusion for 30min.

Inclusion criteria

Inclusion criteria: 1. Elderly patients with fragile brain function include: history of chronic stroke, transient ischemic attack, cerebral infarction lesions with clear medical images (including lacunar infarction), moderate to severe craniocerebral vascular stenosis, Alzheimer's disease, Parkinson's disease, etc. 2. age 65-80 years old 3. ASA classification: II ~ III 4 .expected operation time 2-5 hours

Exclusion criteria

Exclusion criteria: ? long-term use of psychotropic drugs ? alcoholism, drug dependence ? heart, lung and major organ dysfunction ? second surgery ?MMSE score <23

Intraoperative dosage of propofol, sufentanil, remifentanil, operation time, anesthesia time, postoperative extubation time, recovery room residence time, and total postoperative hospital stay.;HR, MAP, SPO2 of patients at different time points;
