Inclusion criteria

Inclusion criteria: (1) Age range of 18-90 years, both males and females; (2) Agree to participate in a 12-month intervention program and commit to not withdrawing from the study during the next 12 months because of extended leave, going abroad, or other reasons; (3) Has assessed the patient's suitability for a 12-month lifestyle change program; (4) Have signed a written informed consent form.

Exclusion criteria

Exclusion criteria: (1) Pregnant or lactating women; (2) Patients with severe cardiovascular complications such as chronic systolic heart failure in the decompensated stage (NYHA cardiac function class =3) and cardiogenic shock; (3) Patients with severe gastrointestinal diseases, liver and kidney failure; (4) Cancer patients or patients who have received radiation therapy or chemotherapy within five years; (5) Patients with infectious diseases such as AIDS and tuberculosis; (6) Those who are hospitalized or suffering from other serious diseases requiring immediate hospitalization; (7) Bad habits: drug use or alcoholism (alcohol intake >80 grams/day for men and >40 grams/day for women); (8) Persons with impaired consciousness, inability to communicate normally, or any mental illness; persons who are unable to take care of themselves or have limited mobility; (9) Persons with a life expectancy of <12 months;
