Interventions

Mobile Internet Platform Group:The mobile Internet platform group uploads various scales and data on mobile phones weekly and monthly, and analyzes and gives feedback on the platform
At 1, 3, and 6 months of follow-up, blood tests were carried out in the most convenient form and uploaded to the platform autonomously
The cardiac ultrasound was reviewed at 6 months and the data was uploaded to the mobile phone
Conventional group:1, 3, and 6 months of outpatient follow-up, according to the condition of the examination and laboratory tests, adjust the drug.

Inclusion criteria

Inclusion criteria: 1.Hospitalized patients with acute decompensation of chronic heart failure had NYHA III-IV cardiac function, EF 25-40%; 2.Age 18-75 years, consent to participate in the study; 3.Be able to complete the mobile data filling index independently or with the help of family members; 4.Be free to move.

Exclusion criteria

Exclusion criteria: 1.Combined with malignant tumor and life expectancy <2 years; 2.Complete bed rest; 3.Did not agree to follow up or could not complete the data submission; 4.Severe hepatic and renal insufficiency; 5.Had a stroke or major surgery within the last 3 months that caused physical impairment; 6.Perinatal woman.
