Inclusion criteria

Inclusion criteria: (1) Age =60 years old and meet the diagnostic criteria for T2DM(at least 6 months).The diagnostic criteria are as follows: Fasting plasma glucose (FPG) = 7 mmol/L, 2-hour post-load glucose(PPG) or random blood glucose (RPG) = 11.1 mmol/L and HbA1c = 6.5%(Group., 2022). (2) Chinese citizen, able to understand Chinese. (3) Treatment with at least one oral hypoglycemic agent and control of HbA1c level of = 7.0%. (4) No cognitive impairment (the confirmation will be based on the patients’ medical records). (5) The patient or family member has a smartphone with internet access and can use WeChat.

Exclusion criteria

Exclusion criteria: (1) Combined with serious heart, lung, kidney and other diseases (the patients’ medical record showed a definite diagnosis). (2) There are obvious barriers of consciousness, unable to communicate effectively. (3) Those who are participating in other clinical trials.
