Inclusion criteria
Inclusion criteria: ? Age = 60 years old; ? has been diagnosed by the second level and above hospitals with at least one chronic disease (cardiovascular and cerebrovascular diseases, respiratory diseases, diabetes and other chronic diseases); ? taking at least two kinds of drugs for half a year and above; ? hospital length > 3 days and in the pre-discharge period; ? have a smart phone and can use WeChat; ? Fuzhou City, a permanent resident (residency = 6 months), the last three months no travelling abroad, travelling plan in the past 3 months; ? no communication barriers.
Exclusion criteria
Exclusion criteria: ?Cognitive impairment (SPMSQ = 3 on the Brief Mental Status Questionnaire), or obvious difficulties or errors in answering the questions; ?Critically ill and/or in the end stage of the disease;?Serious psychiatric illnesses such as schizophrenia, delusions, and bi-directional affective disorders;?Those who have received or are receiving similar interventions; ?Those who are unable to perform basic daily activities independently and need assistance with their medication.
