Inclusion criteria
Inclusion criteria: 1. Inpatients with chronic diseases; 2. Patients who took five or more different kinds of drugs at the same time during hospitalization; 3. The age is 18 years and above; 4. Informed consent and willingness to cooperate with this study.
Exclusion criteria
Exclusion criteria: 1. Patients with severe malignant tumors; 2. Patients whose clinical pharmacists are unable to perform medication reorganization within 48 hours; 3. Due to the patient's special circumstances, it is not possible to inquire about their past medication history; 4. Refuse participants.
