18 Years to 85 Years

Inclusion criteria

Inclusion criteria: 1.Individuals aged =18 years with at least one of the following metabolic indicator abnormalities: Elevated glucose levels: fasting blood glucose =6.1mmol/L and/or HbA1c =6.5% and/or a history of diabetes; Dyslipidemia: total cholesterol =6.2mmol/L and/or triglycerides =2.3mmol/L and/or low-density lipoprotein cholesterol =4.1mmol/L; Elevated uric acid: blood uric acid =540mmol/L and/or hyperuricemia and/or a history of gout or gout attacks; Blood pressure: systolic blood pressure = 140 mmHg and/or diastolic blood pressure =90mmHg and/or a history of hypertension; 2.Capable of performing light to moderate physical activity; 3.No significant cognitive or communication impairments; 4.Permanent residents of Beijing; 5.Signed informed consent.

Exclusion criteria

Exclusion criteria: 1.Currently participating in other drug clinical trials or interventional studies. 2.Individuals with a history of atherosclerotic cardiovascular disease (ASCVD): coronary heart disease, such as myocardial infarction, angina pectoris, and coronary artery stenosis; cerebrovascular disease, such as transient ischemic attacks, ischemic stroke, and carotid artery stenosis; peripheral artery disease. 3.Other serious cardiovascular diseases: such as aortic diseases, cardiac valve diseases, etc. 4.Severe other chronic diseases: heart failure, chronic kidney disease (stage 3 or above), cancer, etc. 5.Difficulty in cooperation (mental illness, hearing impairments, etc.). 6.Other patients who are not suitable for enrollment.
