Inclusion criteria

Inclusion criteria: Inclusion criteria for recruitment of participants are of the Hong Kong older population. 1. 60 – 100 years old; 2. Community- dwelling older adults 3. Identified as having medium or high risk through the screening survey

Exclusion criteria

Exclusion criteria: Exclusion criteria includes: 1. If individuals have significant and unstable medical and psychiatric conditions 2. Having cognitive impairment
