60 Years to 100 Years
Inclusion criteria
Inclusion criteria: 1) Age = 60 years; 2) Diagnosed by the hospital with index diseases hypertension or diabetes: Hypertension diagnosis refers to the "2020 Guidelines for the Diagnosis and Management of Hypertension", and diabetes diagnosis refers to the "Chinese Expert Consensus on the Typing Diagnosis of Diabetes"; 3) Barthel Index score = 40 points; 4) Presence of one or more other chronic health problems besides the index diseases, with clear clinical chronic disease diagnoses; 5) Patients are aware of their own disease diagnoses; 6) Clear consciousness, normal cognitive function, and no communication barriers with the investigators; 7) Patients voluntarily participate in this study and sign an informed consent form.
Exclusion criteria
Exclusion criteria: Elderly hospitalized patients with comorbidities who are in the acute phase of a major physical illness at the time of the survey, or whose condition is critical and requires continuous monitoring
