Experimental group:Esketamine (initial dose 0.5mg/kg, maintenance dose 0.125mg/kg/h)
Control group:Saline (same syringe, same injection method and same injection speed)

Inclusion criteria

Inclusion criteria: 1. Patients who underwent unilateral TKA surgery under combined spinal epidural anesthesia and needed to use a pneumatic tourniquet during the operation; 2. Elderly patients aged 60-90 years old; 3. The American Society of Anesthesiology was graded I-III;

Exclusion criteria

Exclusion criteria: 1. NYHA score = grade III; 2. Child Pugh score = grade B; 3. Chronic kidney disease = stage 3; 4. Severe chronic obstructive pulmonary disease and asthma; 5. SpO2 10×10^9/L) 8. Abnormal coagulation function; 9. BMI > 30kg/m^2; 10. Neurological and mental diseases; 11. Patients who were allergic to any experimental drugs; 12. Patients participating in other clinical trials.
