intervention group:Three times of online video education, two times of phone follow-up a week and eight times of wechat knowledge push for a period of 6 weeks.

45 Years to 80 Years

Inclusion criteria

Inclusion criteria: (1) have dementia, cognitive assessment results show mild or severe dementia; (2) At least one cardiovascular disease (including high blood pressure); (3) suffering from cardiovascular disease for not less than 6 months; (4) Can speak and understand Chinese; (5) The ability to express voluntary participation in research.

Exclusion criteria

Exclusion criteria: Have a life expectancy of less than 12 months, live in a nursing home or long-term care facility, have no family caregiver with whom they maintain contact, or are participating in other research involving the management of chronic diseases.
