31 Years to 80 Years
Inclusion criteria
Inclusion criteria: (1) Aged 18 years and above; (2) patients who have been diagnosed with T2DM for at least one year; (3) enrolled in the community health service center's chronic disease management system for at least one year; (4) take at least one oral hypoglycaemic drug for treatment or undergoing insulin therapy; (5) no cognitive problems; stable condition; ability to self-manage their lives and willingness to cooperate; (6) ability to listen, speak, read, write, etc., and ability to perform basic exercises required for treatment; (7) ability to use a mobile phone to receive and read text messages; (8) informed of the study purpose and willing to participate and sign an informed consent form.
Exclusion criteria
Exclusion criteria: (1) Diabetes with serious complications, such as severe diabetic nephropathy, severe diabetic retinopathy, etc.; (2) with other serious physical diseases; (3) patients with mental disorders and cognitive impairments; (4) patients concurrently diagnosed with cancer and having received chemotherapy, radiotherapy or other treatments within the past six months; (5) suffering from special types of diabetes or other diseases not suitable for this study; (6) patients who are participating in other research projects.
