Inclusion criteria
Inclusion criteria: 1. aged 18-80 (inclusive) 2. voluntarily download the chronic disease data management software system app for joint care and sign the informed consent 3. can come to the common care clinic = 2 times a year 4. can cooperate with doctors to implement medical orders, no serious disease, and life expectancy = 15 years
Exclusion criteria
Exclusion criteria: 1.patients with important organ failure or other serious diseases, including myocardial infarction, severe mental disorder, severe infection or disseminated intravascular coagulation within one year before enrollment; 2.patients with active or inactive malignancies and life expectancy less than one year; 3.patients with communication barriers who cannot fully understand or cooperate; 4.pregnant women, lactating women and women who have a pregnancy plan in the past year;
