31 Years to 80 Years
Inclusion criteria
Inclusion criteria: 1) At least 1 year of inclusion in the chronic disease management system of community health service centers; 2) T2DM patients diagnosed for 1 year or more; 3) =18 years old; 4) Be able to perform the basic exercises required for treatment; 5) Take at least one oral hypoglycemic drug or use one insulin for treatment; 6) Clear consciousness, stable condition, no cognitive problems; 7) Have the ability to read, write, etc., and be able to communicate effectively; 8) Ability to use mobile phones and read text messages; 9) Be willing to participate and sign an informed consent form after being informed of the purpose and content of the study.
Exclusion criteria
Exclusion criteria: 1) Patients with mental and cognitive disorders; 2) Concomitant diseases that are not suitable for or affect this study; 3) Patients who have received chemotherapy, radiotherapy and other treatments and have tumors in the past six months; 4) Unwilling or unable to provide informed consent; 5) Patients who are currently involved in other research topics
