Inclusion criteria

Inclusion criteria: Primary care physicians who work full-time in rural health centers and clinics are eligible to participate. The hypertensive patients should meet specific inclusion criteria. a) Age from 45 to 75; b) The patients with physician-diagnosed essential hypertension; c) Local residents who have been living in the city for over one year.

Exclusion criteria

Exclusion criteria: a) Have a physician-diagnosed stroke, myocardial infarction, heart failure, malignancy, regular kidney dialysis, or end-stage renal disease; b) Have a history of secondary hypertension diagnosed by a physician; c) Women who are pregnant, breastfeeding, or planning to become pregnant within six months; d) Have the intention to leave your residence within six months.
