General Management Group:According to current relevant policy management, patients will receive management services within the regular basic public health service system after discharge

Inclusion criteria
Inclusion criteria: 1. Residents with permanent residence registration in the region (1 year residence in the region for more than 6 months); 2. There is no plan to emigrate or work outside within 2 years; 3. Voluntary participation and signing of informed consent; 4. Initial acute ischemic stroke, confirmed by computed tomography or magnetic resonance imaging, hospitalized within 72 hours after onset; 5. There were one main nervous system symptoms and signs (hemiplegia involving more than two body parts, numbness involving one side of more than two body parts, homonymous hemianopia, aphasia) or two secondary nervous system symptoms and signs (diplopia, vertigo or gait abnormality, dysarthria or dysphagia or dysphonia) and lasted for more than 24 hours.
Exclusion criteria
Exclusion criteria: 1. Unable to sign a consent form; 2. Malignant tumor undergoing treatment and life expectancy (to judge by the professional doctor) < 2 years; 3. Pregnant women, are trying to conceive, or may have fertility without contraception measures; 4. psychopath.
